Hip / waist pain — full detail
What happened
Onset early–mid April 2026, during a Bulgarian split squat with an awkward lateral shift. The spot: left lateral pelvis / iliac crest / waist, just above the hip bone — running around the side, front and back. Higher than the buttock or the greater trochanter (the bony point of the hip joint).
Character: mild, intermittent soreness / tightness / knot-like feeling. Not debilitating, and it has NOT substantially progressed in ~6 months — with quiet periods and flare-ups.
What triggers it — and what doesn't
- Triggers: bending forward, coming back up from bent-forward, seated pelvic circles, stairs and other asymmetric loading, kneeling / lowering down.
- Does NOT trigger: side bending, backward bending.
- Quiet signs: normal walking generally comfortable; no numbness, tingling or weakness; no shooting leg pain; never wakes him from sleep.
Relevant history: left tibia + fibula spiral fracture ~2020, surgically repaired with a retained intramedullary rod; occasional left knee discomfort with squats; longstanding limited flexibility (can't touch toes). No diagnosis, no imaging, no physio so far.
The advice
One in-person session with a sports physiotherapist BEFORE any imaging. The exam that matters: palpation of the crest itself plus watching how the movement splits between lumbar spine and hip when he hinges — that distinguishes a crest/QL-type soft-tissue problem (trainable) from something that needs a scan.
Exercises alone were suggested back in April (glute bridges, clamshells, step-downs) and were never done in a structured way — so "just do exercises first" has already effectively been the plan for six months, without the exam. The valoración comes first; the physio then sets the exercise dose.

Muchas gracias.
Exercises on record (from April)
- Glute bridges
- Clamshells
- Step-downs (controlled, pain-free range)
Suggested in April, never structured. Treat these as background, not the plan — the valoración sets the real program.
